Uriel Therapy’s Commitment to Clients

• All clients must complete a consultation form prior to treatment and will receive 15 mins + consultation prior to their first treatment, and before some subsequent treatments if required.

• Health & safety instructions pertaining to laser therapy will be given.

• Treatments will only be conducted if deemed safe for the client to receive.

• GP/Consultant permission may be required before treatments can be provided

Payment

• An invoice will be emailed after 6 sessions or at the end of the month. Bank details will be given for payment by bank transfer..

• Payment can be made by cash or cheque but regretfully not by credit/debit cards.

• All invoices must be paid within 7 days of issue date.

• If payment is not received it will be expected before the next appointment.

Cancellation Policy

• A booking is confirmed once Uriel Therapy and the client have both agreed a date, time, duration and location for a treatment

• If a cancellation is made more than 24 hours before a treatment, the client will not be charged

• If a cancellation is made within 24 hours of the treatment, the client will be expected to pay a cancellation fee of £30 for each 30 mins cancelled

• This cancellation fee may be waived at Uriel Therapy’s discretion.

• The cancellation fee must be paid within 7 days of the cancellation

• Uriel Therapy has the right to cancel a treatment by giving 24 hours’ notice

• Uriel Therapy has the right to cancel a treatment without notice if the room or environment is unsuitable for the treatment to be provided, or if the therapist is taken ill.

• Uriel Therapy has the right to terminate a treatment immediately if the client demonstrates inappropriate  behaviour

• Uriel Therapy commits to rearranging treatments to a new time and date within two weeks of the cancelled treatment where possible

                                                                                          Infectious disease precautions

Should we enter Covid or other infectious disease restrictions please cancel your appointment if you are unwell or have been requested to self-isolate.

All clients will be asked to wear a mask, remove their shoes and gel their hands prior to entering the treatment room.

All surfaces will be wiped down with disinfectant wipes after each client.

Under all circumstances please cancel your appointment if you have symptoms of vomiting and diarrhoea within 48hours, or symptoms of influenza as I treat vulnerable people.

You will not be charged a cancellation fee for a short notice cancellation due to illness.

• Client treatment details will not be discussed with anyone other than the client unless the client is under the age of 18 or has a care worker or guardian

• The details of the treatment only may be discussed with a medical doctor at their request, and only after permission is obtained from the client.

• Consultation Forms and treatment notes are kept in paper format only and will be stored securely.

• All clients must complete a consultation form prior to receiving treatment.

• By signing the consultation form, clients are giving Uriel Therapy permission to hold records and data about them

• Personal information will not be shared with any third parties

Young Clients

• Clients under the age of 18 must be accompanied by an adult

• The adult must give permission for the treatment to be performed

• The adult must co-sign the client’s consultation form

• The adult must be present in the room throughout the treatment

Qualifications, Insurance and Ethics

Margaret Collins 

• Is registered with K-LaserUK

• Abides by the safety and ethical codes provided by K-LaserUK

• Is fully insured for classical & deep tissue massage and class IV laser therapy

• Holds a iTEC Level 3 Diploma in Massage. Grade Distinction. Qualification number 603/4092/7 Awarded 17/09/2020

• Qualified in Veterinary Medicine & Surgery from Cambridge University in July 1986 MA VetMB MRCVS

Complaints Procedure

• If a client wishes to make a complaint about Uriel Therapy this must be addressed to Margaret Collins and emailed to urieltherapy@gmail.com

• Complaints must include the date and location of the incident, the full name of the complainant, details of the complaint and a desired outcome following the complaint

• All complaints will be taken very seriously and a response will be provided within 28 days
